Before accessing our registration forms, please read the following directions/information regarding registering to scheduling an appointment.

We need to receive and process your registration paperwork to schedule an appointment.

Please print or download and create a PDF, then complete our intake registration paperwork. Return via FAX or USPS.

We will call you to schedule an appointment after your registration has been processed.

If you have a history of skin cancer, please request your previous dermatologist send

Dr. Endo your last three (3) years of dermatology visit notes and all pathology reports.

• We continue to minimize in office contact; we only allow patients in the office on the date and time of their appointment.

• If the patient needs physical assistance or the patient is a minor; we do allow one other person to accompany the patient.

• ​It is important that you verify/confirm that we participate with your insurance prior to your appointment.

• It is the patient's responsibility to understand their coverage/benefits/co-payments/deductible.

• Please contact your insurance carrier if you have any questions.

• Insurance cards  are required at every office visit. We verify coverage at every office visit.

• ​If you have an HMO plan it is the patient's responsibility to request a referral prior authorization from your primary care physician.

• ​​Masks are optional. We respect your choice.​

• Returning forms:  We are unable to receive paperwork via email or text; it is not secure, nor HIPAA compliant. Protecting your personal data is important to us.

If you should need to change or cancel your appointment, kindly call us at least 24-48 hours in advance to avoid a possible $40.00 late cancellation fee.

This also allows us to call any patients on our wait list.

Appointment reminders are not guaranteed.

Please write/save your appointment to your calendar. Missed appointments may result in a  $40.00 "No-Show" fee being billed to you.
